Dear Patients and Community Members,
I am writing to share an important update regarding Live Well Physical Therapy, LLC. After thoughtful consideration, I have made the decision to cease accepting new patients, with final patient care concluding at the end of March.
I opened Live Well Physical Therapy, LLC in June 2022 with the goal of serving our community through personalized, high‑quality physical therapy. As my academic role has continued to evolve, I’ve recognized that maintaining both my full‑time education work and my clinical practice is no longer sustainable long‑term. This decision reflects my commitment to creating healthy boundaries, supporting personal well‑being, and ensuring I can focus forward. With this in mind, I have made the decision to close Live Well Physical Therapy.
Throughout this journey, I’ve been honored to provide care not only through physical therapy, but also through wellness and performance services—always with a passion for helping people keep moving in the ways they know best and live their lives with less pain.
To support the possibility of continued service to the community, the business is available for sale. My sincere hope is that someone with a passion for patient‑centered care may wish to carry forward what has been built.
I want to extend my deepest gratitude to all of my patients and clients. Thank you for trusting me with your care and allowing me to be part of your health and wellness journey. It has been an honor to serve you.

Business Model
Live Well Physical Therapy is an out-of-network (cash-based) clinic, which means we do not accept insurance. Instead, we focus on what truly matters—you and your care. We cannot see Medicare or Medicaid patients for payment.
We accept HSA cards, credit cards, checks, and cash. For patients with out-of-network benefits, we provide a detailed invoice (superbill) that you may submit to your insurance provider for possible reimbursement.

Vestibular Rehabilitation – Ottawa Clinic & Hancock County Mobile Care
At Live Well PT, Dr. Colbert and Dr. Anderson provide comprehensive vestibular rehabilitation both at our Ottawa clinic and through mobile care in Hancock County. This specialized therapy supports individuals experiencing vertigo, dizziness, imbalance, and related symptoms that affect everyday life.

At Live Well PT, Dr. Anderson and Dr. Colbert provide comprehensive concussion rehabilitation to help individuals recover from the complex and often frustrating symptoms associated with head injuries. Whether caused by a fall, motor vehicle accident, sports injury, or another event, concussions can impact multiple body systems—including balance, vision, cognition, and sleep.

Dr. Colbert treats across the lifespan as a ABPTS Board Certified Neurologic Specialist with extensive experience caring for patients with neurologic deficits and mobility challenges.

This screen includes a 30-minute focused, one-on-one musculoskeletal screen, guidance on next steps, and personalized recommendations. Appropriate if you have lingering pain, a new injury, or movement limitations you are concerned about. Sometimes a little advice can go a long way. Call to schedule.
